Just follow these three simple steps:

1. See your Benefit Administrator for the appropriate enrollment form.
2. Estimate your annual unreimbursable health-care/dependent care related expenses. Use the handy Worksheet Estimate Form.
3. Return completed enrollment form to your Benefit Administrator.
