PROTECTING YOUR PRIVACY AT ADVANCED BENEFIT STRATEGIES
We are required by law to maintain the privacy of your health information and provide you with this notice.

It informs you how we handle personal information. This notice applies to: employee benefit Section 125 Pre tax Benefit plan participants and beneficiaries.

Please note that “you” refers to only these people.

WE PROTECT INFORMATION WE COLLECT ABOUT YOU
We follow strict standards to protect personal information. These standards include limiting access to data, using technology to assist with security and complete document destruction prior to disposal.

HOW WE COLLECT INFORMATION
We collect data ABOUT YOU AS WE DO BUSINESS WITH YOU. Some of the sources of the data are as follows:

  • Information we obtain when you enroll for services in a Section 125 Benefits Plan. You may provide facts such as your name, address, Social Security Number, annual salary, and other information considered personal.
  • Information we may obtain from others may include insurance claim reports, pharmacy receipts and similar data.

MEDICAL INFORMATION
We do not use or share medical information internally or externally for any purpose other than the administration of your account or claim. Exceptions to this may be:

  • When needed to service your account, claims or contracts.
  • When laws protecting your privacy permit it.
  • When you consent.
  • In response to a subpoena.
  • To prevent fraud and or for other legal purposes.

ACCURACY OF INFORMATION
We strive for accurate records. Please tell us if you receive any material from us containing incorrect information. We will make the appropriate changes.

You may write to us if you have questions or would like another paper copy of our Privacy Notice. Submit your request, Attention Privacy Officer at Advanced Benefit Strategies, 30 Mill Street, Unionville, CT 06085. We may change our privacy practices at any time. We will give you a revised notice when required by law. Our privacy practices comply with all applicable laws. If a state’s privacy laws are more restrictive than those stated in this Notice we comply with those laws.

EFFECTIVE DATE
This Notice is effective April 14, 2003 for “large” Health Plans or April 14, 2004 for “small” Health Plans.
